We ask that you allow 2 full working days for a repeat prescription request to be completed, although the practice will always aim to complete these as soon as possible.
Please tick the appropriate medication you require from your last prescription and either drop it in to reception or in the post box outside the main entrance. If you are unable to find your last prescription form you can either come into reception and request a form which you can fill out, or drop us a letter with your name, address, date of birth and the requested medication. You may send us a self-addressed envelope with your request but please allow five working days. Your prescription request must always be in writing; no requests will be taken over the phone.
When your medication needs review we will put a note to this effect on your prescription. If your prescription goes directly to the pharmacy please check with the pharmacist for any messages on your prescription. Please make the appropriate appointments as requested.
If you require an early issue of your prescription e.g. for holidays please advise us on your request in order that we can try to facilitate this. All other prescriptions will be issued as they become due.
Please note that due to the volume of repeat prescription requests received, if you need a review with a clinician before your prescription can be issued, the practice WILL NOT CONTACT YOU. We would therefore respectfully suggest that it is everyone’s best interests for repeat prescription requests to be submitted in good time, and not at the last minute.
