News Regarding Dr Smith - July 2019


We are writing to share some sad news with you all. As you may be aware, Dr Smith was off for a period of time earlier in the year. Whilst he returned to work in April, it has become necessary for Dr Smith to make the very difficult decision to retire from the practice due to recurrent illness and he will not be returning to work before this comes into effect.


I'm sure you will all join us in thanking Dr Smith for all the hard work and commitment he has put into making Churchill Medical Centre such a great surgery and in wishing him the long,  happy retirement that he richly deserves.


If any patients wish to send Dr Smith a personal message of thanks, we will be collating these and presenting them to him later in the year.


New Appointment System Software from Wednesday 3rd April 2019

From Wednesday 3rd April the software we use at the practice for our appointment system is changing. This means if you currently log in to our appointment system online or request your medication through the online appointment system you will need to use a new log in.

All currently booked appointments will be carried across in to the new appointment system so there is no need to call to re-book or confirm.

Why are we doing this?

The change in system means all of our appointments and your medical record will be available on one piece of software, EMISWeb. You will then be able to request your prescriptions, book appointments and see parts of your medical records all in one place.  Through this you will also be able to use the NHS App, download the ‘Patient Access App’ or use the ‘MyGP App’ for all online services, making access to your records and the practice much easier.

What does this mean for you?

If you have previously brought in your photographic ID and signed up for appointments online you will need to log in using the ‘Patient Access’ details you may already have.  If you do not have these details, please contact the practice.

An email will be sent to you with your log in details if we have seen your ID.

What if you have not previously brought in any photographic ID to the practice?

You will need to come in to the surgery during our opening hours (8:00am to 8:00pm Monday to Friday, 8.00am to 12.00pm Saturday morning) to receive your sign up details. 

We ask for proof of identity in order to sign you up for online services as you are receiving a log in not just to your appointments, but to your medical record, which contains your personal confidential information.


From 1st October 2016, Kingston CCG additional clinics initiative is offering weekend appointments at Surbiton Health Centre, Kingston Health Centre and The Merritt Centre (Chessington). These appointments are available to patients from ALL Kingston GP practices, and can be booked by calling 020 3841 9942 from 8.00 a.m. to 8.00 p.m. on Saturdays, Sundays and Bank Holidays.The NHS 111 service will also be directing patients to these clinics
